Custom forms
Custom forms are for admins and employees who assign forms to client records or review saved answers. Use them when the firm needs structured answers, uploaded supporting files, or repeatable intake questions outside the standard client fields.
Custom forms are different from Custom fields. Custom fields add extra fields directly to a Tasks or Invoice record; custom forms are standalone questionnaires you attach to a client, task, or service template.

Create a custom form
Section titled “Create a custom form”- Open Settings.
- Open Customization.
- Select Custom Forms.
- Add a form from the Custom Forms list.
- On Add Custom Form, choose Select Form Type.
- Build the questions.
- Select Save Form.
When you update an existing form, the screen is Edit Custom Form and uses the same form type and save controls.
The available form types are:
- Task
- Individual Client
- Business Client
- Service Template
Use Individual Client or Business Client for forms that belong on client records. Task is for task-related forms. Service Template exists as a form type, but service-template administration is outside this article.
Attach a form to a client
Section titled “Attach a form to a client”- Open Clients.
- Add or edit an individual or business client.
- Open the Custom Forms step.
- Select Add New Custom Form.
- Enter Version/Year.
- In Add Required Forms, select the form.
- Complete the form answers.
- Save the client form.
The selected form is saved with that client. Use the version or year to distinguish annual forms, intake versions, or updated firm questionnaires.
Review and update answers
Section titled “Review and update answers”On the client’s Custom Forms step, saved forms appear as expandable rows. Open a row to review the saved answers. Use the pencil icon to edit a saved form. Use the trash icon to remove a saved form when more than one form is attached and your role allows deletion.
Read-only form views include Save as PDF. Use it when you need a PDF copy of the saved answers for review or filing.
File-upload questions
Section titled “File-upload questions”Custom forms can include file-upload questions. Files uploaded through a form answer stay with that form response. For general documents that should live in the client’s document folder, use Client files instead.
Missing controls
Section titled “Missing controls”If Custom Forms, Add Custom Form, edit, delete, import, export, or status controls are missing, ask an administrator to review your access in Permissions and Visibility.
